Join MHCH Think Tank Day to develop creative care pathways, launch the Aftercare Portal, and collaborate on solutions for children.
You are invited to join the Mental Health Collaboration Hub community for a Think Tank Day where we convene around developing Creative Care Pathways for children experiencing boarding, launch the Aftercare Portal, as we continue to grow our collaboration around solutions for children and families.
We will gather on Tuesday, Oct. 6th in Saint Paul at the Fairview Community Health and Wellness Hub (45 W 10th St., St. Paul, MN 55102) from 10:00 am - 3:00 pm and will:
Start with a shared understanding of the current state of boarding for children
Explore creative solutions for children to access community-based care through the Creative Care Pathways funding
Launch within this community the Aftercare Portal to support aftercare planning for children transitioning from intensive residential/hospital/related settings to home
We will also enjoy connecting with professionals working across the children and family service continuum who want to collaborate to be part of building these solutions for children and families.
All who see themselves as part of this work are welcome.
